UMR-administered behavioral health benefits cover outpatient rehab and mental health treatment, and Quest Wellness Center works with those benefits for PHP, IOP, virtual IOP, and outpatient care for adults. Because UMR plans are built by each employer, our admissions team checks what your specific plan includes in about 15 minutes, with no obligation to enter treatment.

What Is UMR?

UMR is a third-party administrator owned by UnitedHealthcare, not an insurance company you buy a policy from. It processes claims and administers behavioral health benefits for employer-sponsored health plans, and it uses the UnitedHealthcare provider network, so the providers available to you are generally the same ones available to UnitedHealthcare members. If your insurance card says UMR, your benefits are administered by UMR and funded by your employer’s plan, and Quest Wellness Center works with that benefit when we verify your coverage.

Because each employer designs its own plan, two people who both carry UMR cards can have very different benefits. That is why verification matters more with UMR than with a standard carrier: the only reliable way to know what your plan includes is to check your specific employer plan, which our admissions team does with you before you start. Some UMR plans administer behavioral health through Optum, and we confirm how your particular plan is set up during verification.

Outpatient Levels of Care Available to UMR Members at Quest Wellness Center

UMR members at Quest Wellness Center can use their behavioral health benefits across the full range of outpatient care, from the most structured day programming down to lighter ongoing support. Each level is built around your clinical needs and your schedule, with morning, afternoon, and evening tracks available.

  • Partial Hospitalization Program (PHP): our most structured outpatient level, running 5 to 6 hours per day, 3 to 5 days per week.
  • Intensive Outpatient Program (IOP): structured therapy running 3 to 4 hours per day, 3 to 5 days per week, while you live at home.
  • Outpatient Program (OP): lighter ongoing support, about 5 to 7 hours per week.
  • Virtual IOP and online therapy: the same programming delivered securely from home.
  • Dual diagnosis care: integrated treatment when a mental health condition and substance use occur together.

How to Verify Your UMR Benefits With Quest

Verifying your UMR benefits with Quest Wellness Center takes about 15 minutes by phone or about 24 hours through the form on this page. Here is how it works.

What UMR Covers for Outpatient Treatment

UMR-administered plans include benefits for substance use and mental health treatment. Federal law requires it: the Affordable Care Act names addiction and mental health services as essential health benefits, and the Mental Health Parity and Addiction Equity Act requires these benefits to be treated on par with medical care. UMR plans comply with these protections regardless of which employer designed them.

Quest Wellness Center works with UMR-administered benefits for the outpatient levels of care we provide directly: partial hospitalization (PHP), intensive outpatient (IOP), virtual IOP, standard outpatient, and dual diagnosis care for co-occurring mental health and substance use conditions. Which of these your benefits include depends on your employer’s plan, which is why our admissions team verifies them with you before you start.

Conditions We Treat Under Your UMR Plan

Quest Wellness Center treats both mental health conditions and substance use under UMR behavioral health benefits. Our programs are led by licensed and registered clinicians using evidence-based methods including CBT, DBT, EMDR, ACT, and group and individual therapy.

Mental health conditions we treat include depression, anxiety, post-traumatic stress disorder and complex trauma, personality and mood disorders, and suicidal ideation. Trauma-focused care includes EMDR therapy within our PHP and IOP programs, while CBT and DBT address the thought and behavior patterns that drive anxiety and depression.

On the substance use side, we treat alcohol, opioids including prescription opioids and fentanyl, cocaine, methamphetamine, prescription drug dependence, and behavioral addictions such as sex and gambling addiction. When a substance use condition and a mental health condition occur together, our dual diagnosis program treats both at the same time under your UMR behavioral health benefit, because treating only one rarely produces lasting recovery.

What If Your UMR Plan Doesn't Cover Everything You Need

If your UMR plan does not cover a service you need, our admissions team works through your options with you before you make any decision. Some Quest programs may suit your plan better than others, and we help you find the right starting point. For care we do not provide directly, such as medical detox or residential treatment, we coordinate a referral to an appropriate provider. We will always walk through what is workable with you rather than leave you to figure it out alone.

Frequently Asked Questions

Is UMR the same as UnitedHealthcare?

UMR is a third-party administrator owned by UnitedHealthcare, not a separate insurance company. It administers employer-sponsored plans and uses the UnitedHealthcare provider network, which is why your card may reference both. Quest works with UMR-administered benefits, confirmed during verification.

UMR is not insurance you buy directly; it is the administrator that processes claims and manages benefits for an employer-funded health plan, under UnitedHealthcare. Your employer designs the plan, so your specific benefits depend on that plan. We confirm what yours includes in about 15 minutes.

Yes. Partial hospitalization (PHP) is typically covered under UMR-administered behavioral health benefits, and Quest works with UMR for our PHP. We verify the specifics of your plan before you start.

Yes. UMR-administered plans cover telehealth behavioral health services on most plans, and Quest offers virtual IOP and online therapy with the same programming as in-person care. We confirm your telehealth benefits during verification.

Some UMR plans require prior authorization for certain levels of outpatient care, and others do not. Our admissions team checks whether your employer’s plan needs it and handles that step with you as part of admissions.

Why Choose Quest Wellness Center With Your UMR Plan

Quest Wellness Center pairs UMR-administered behavioral health benefits with accredited outpatient care built around your schedule. We are accredited by the Joint Commission and licensed by the California Department of Health Care Services, two credentials you can verify independently, and our team serves adults in person and through secure telehealth.

Our outpatient model is designed to fit treatment around your life rather than pause it. PHP, IOP, and outpatient care all run on morning, afternoon, and evening tracks, so you can keep working, studying, or caring for your family while you receive structured clinical support. Every program combines individual therapy, group therapy, and evidence-based modalities including CBT, DBT, EMDR, and ACT, delivered by licensed and registered clinicians.

Because we treat mental health conditions and substance use together, UMR members with co-occurring conditions do not have to coordinate between separate providers. Our admissions team handles your UMR benefit check, explains which programs your plan includes, and matches you to the right level of care, all before you commit to treatment.

All three levels are available to UMR members at Quest and use the same evidence-based therapies; they differ mainly in time commitment and intensity. PHP offers the most structure, IOP balances structure with daily life, and outpatient care supports longer-term progress. Our team helps match you to the right level during verification.
